2012 Participation Checklist

Please refer to the checklist below throughout the process to ensure that you are on track with the 2012 program requirements. All program forms, sample reports and surveys are available here. October 2011 Program Registration begins.  See FAQs for information about survey customization and oversampling.  
March 19
Program registration deadline.  Confirmation of registration will be sent via email when the registration is submitted.  Organizations will then receive an invoice within 15 days (if applicable).  Fees are due within 15 days of invoice receipt.  Please contact Karen Kukulka at kkukulka@modernthink.com to discuss survey options.

Deadline to submit customization requests (optional).  See Applicant Information for more details.

A web link for the ModernThink Administrative Portal is sent to the Primary Contact.  The portal is used by applicants to:

* Upload employee email addresses for survey distribution.
Random Sample Generation Instructions
* Request any access codes/login sets for employees without company emails addresses (online option only).
* Order paper surveys.
* Download, complete and upload Survey Verification Forms.
* Customize Job Role and Department (optional).
* Increase survey audience by oversampling (optional).

Pre-Survey Communication sent by your organization

TIP:This letter should be from your organization’s CEO or another senior representative. The letter "positions" the survey/program and serves as an alert so that employees do not delete the survey invitation email. Organizations that pre-communicate the survey typically have significantly higher response rates.

Also, it is recommended that you send an additional reminder to your employees during the survey period. A sample of this communication is also included here.

Paper version of the employee survey and collection envelopes are sent to the Primary Contact at the participating companies for distribution (paper option only).
